Billing Information
At your first visit, we will provide you a welcome package. You can download this package from website before your first visit; completing of this form prior you visit should save your time and allows us to enter information to your electronic chart.
To download the statement please go to “patient center” and click on “forms” and select the form that you need to complete. Please inform our staff if there is any change in your insurance. The Greater Washington Arthritis, Rheumatology and Osteoporosis Center accepts cash and personal checks as well as the following credit cards:
If we are a participating provider with your insurance company, we will bill them. However, payment is the patient's responsibility and our billing company could assist you in handling claims. For billing inquiries, please contact our billing company at (877) 386-3291.
We recommend completing the “New Patient Form” and other relevant Questionnaires Forms for your visit. The initial visit form provides us with valuable information, which is very important for an accurate assessment and diagnosis. Your referral physician provides us the reason for consultation; however, we still need a detailed history of your disease and previous treatments which will prevent unnecessary laboratory tests or imaging studies. When you present to the office, please bring your most up to date medication list, pertinent labs and imaging studies such as CT, MRI and X-ray report. It will be very helpful if you can bring an actual image in any format to be reviewed.
The questionnaires will help us to assess your treatment progress and adjust the medication doses or switch to an alternative medication if it is necessary.
Your initial visit or consultation time with the doctor may vary from 45 to 60 minutes based on the complexity of the case and follow-up visits may range between 15 to 25 minutes.
